Notice Regarding Forms 1095-B and 1095-C

Employees and other covered individuals may request a copy of their Form 1095-B or Form 1095-C for the 2025 tax year.​ To request a copy, please contact the School’s Director of Operations, Caitlyn Nemeth, at cnemeth@jacksonvilleclassical.org.

Forms will be provided to requesting individuals within 30 days of the date the request is received.
